We ask each student we see to fill out a "Student Information Sheet" so that we can keep track of who we see. You case will be stored on our confidential database AdvicePro which can only be accessed by members of the Advice Centre. Your information will not be shared with other members of the Union or the University without your consent.  

Consent to Store Information:  

To ensure the effectiveness of our service we need to keep a record of your enquiry. We also need to keep your details so that we can contact you, if necessary, about your case.  These will only be accessible by Advice Centre staff and student volunteers. Under the Data Protection Act 1998 you have a right to see any records we keep about you.

I give my consent to information regarding myself being held by the Advice Centre on their online database, and confirm that to the best of my knowledge, the details I have given are true.

 

Occasionally it will be necessary for us to communicate with a third party (staff member from the University, external organisation, etc) on your behalf. To do this, we require your consent:

Consent to Share With and Receive Information From Relevant Third Parties:

The UCS Union Advice Centre offers a confidential service run by staff and student volunteers. Unless you have given your permission, nothing you tell us will be shared with any other organisation or individual. This is with the exception of situations where you or another person may be at risk, or if we are required to disclose information by law (see Confidentiality Policy and Procedures for full details).

I give my consent for the staff and student volunteers of the Advice Centre to correspond and share my information with any relevant third party on my behalf for the purpose of dealing with my case, and for these third parties to share relevant information with the Advice Centre in return.
